MCE for children and youths – general profile
From 01.08.2011 we have a contract with the National Health Fund to provide the therapeutic care for children and youth.
Activity
In our company we care for children who have completed the process of diagnosis and treatment in hospital and do not have to stay in there, but because of their poor health and/or disability, they require constant professional medical care, nursing and rehabilitation.
All children are provided with multi-profile treatment, including a variety of educational impacts to restore their efficiency to the maximum possible extent. Doctors and nurses, speech therapists, physiotherapists, occupational therapist and psychologists cooperate with us in this regard.
What are the terms of inclusion?
To place your child in our facility, it is necessary to complete and provide us with the following documents:
- referral from a physician stating that the child needs addressed 24 hour treatment, skilled nursing and rehabilitation, and does not require hospitalization (printing download HERE)
- filled Barthel scale (printing download HERE)
- interview conducted by a nurse or a public health nurse in a hospital ward - if the child is currently in hospital (printing download HERE)
- copies of the information from the existing hospital
- original vaccination card
- written authorization for any additional hospital treatment, nursing care and vaccination (compulsory and supererogatory)
- current results: ESR, CBC, urine (examination), chest X-ray
- written consent to the placement of the child in the MCE signed by a parent or legal guardian (printing download HERE)
- evidence of insurance with the National Health Fund
- child's birth certificate with a stamp on the back, confirming his permanent residence address and social security
- photocopy of parent’s or legal guardian’s ID card
- court document stating the establishment of a legal guardian
- statement of earnings
- parent’s or legal guardian’s contact details
